Wellness Visit

Everyone, at every age, deserves a yearly wellness visit. This is your annual checkup to review your overall health, update immunizations, order recommended screenings, and make sure you and your provider are on the same page about your health goals.

What to Expect

Bring a list of your current medications, any health concerns, and your insurance card. No special prep is needed unless labwork is ordered in advance. After your visit you may be referred for additional screenings or follow-up appointments.

What's Included

Physical exam, growth and development review, medication review, immunization updates, and referrals for age-appropriate screening.

What's Not Included

Treatment of new or urgent symptoms, specialist consultations, and diagnostic imaging. These may require a separate visit.

Chronic Disease Management

Managing a chronic condition takes consistent, ongoing care. These visits are typically scheduled every three months to monitor your health, review lab values, adjust medications, and make sure your condition is well controlled.

What to Expect

Visits are scheduled based on your stability and condition. Bring your current medications and any notes about symptoms or side effects you have noticed. Labwork may be ordered before or after your visit.

What's Included

Medication review and adjustments, lab value monitoring for conditions such as diabetes, hypertension, and high cholesterol, side effect discussions, and care plan updates.

What's Not Included

Specialist care, urgent or same-day sick visits, and procedures outside the scope of primary care management.

Women's Health

We provide comprehensive women's health services right here in our clinic. From routine pap smears and pelvic exams to breast health and STD testing, we are here to support your health at every stage of life. We are proud to partner with Breast Care to help those without insurance or who are underinsured access the care they need.

What to Expect

For pap smears, avoid intercourse, douching, or vaginal medications for 48 hours before your visit. Wear comfortable clothing. Your provider will walk you through every step so you feel informed and at ease.

What's Included

In-clinic breast exams, self breast exam education, mammogram and diagnostic imaging referrals, pap smears, pelvic exams, STD testing and treatment, and Breast Care partnership resources for uninsured or underinsured patients.

What's Not Included

Obstetric care, contraceptive procedures, and gynecological surgeries. These services may require a specialist referral.

TeleHealth

Can't make it in? No problem. We offer video visits for our established patients so you can connect with your provider from wherever you are, whether you are traveling, under the weather, or simply unable to come in.

We understand that not everyone can make it to the clinic. For our elderly patients and those with limited mobility or special circumstances, we offer home visits to bring care directly to you.
